delete image with checkbox check than click on save button

good morning,
i have this problem: in a joomla component i have a checkbox that if checked it can remove the image or file that i previously upload on input field.
Here the code:

The problem is: when i check the checkbox and save button, the page refresh but the image still stay there.

Would you be able to format your code to make it more readable? Right now I'm not seeing the button code. Also you should know that there is a dedicated joomla site Joomla.
